Wiki Coding CAD s/p CABG

If documentation states that a patient has CAD s/p CABG, would we only use the code for the CAD since the code book doesn't direct you to use an additional code for the CABG? Is that included in the code for CAD?

If it is documented by the provider as relevant to the encounter, you should code it - s/p CABG would be reported with Z95.1.
