I wouldn't unless the condition is also documented by the attending physician.
 
Is it appropriate to code all conditions listed on the anesthesiologists pre-anesthesia evaluation prior to surgery, since those things are being considered before placing the patient under anesthesia even though they are not being treated at that time and may not relate in any way to the reason for surgery?

I would code all the listed conditions. The anesthesiologist may need to consider those conditions when administering the anesthesia. I don't believe it has anything to do with the reason for surgery nor if they are being treated.
