Hello everybody, I have a question, are we suppose to code chronic conditions (hypertension,diabetes, ect...). I was informed I should be looking at the H&P and coding chronic conditions off the H&P even though if the patient has a condition but was not documented on the surgery op note and their condition did not have any affect on the surgery. Input would be greatly appreciated.
