Is it an allowable/appropriate practice to report ICD 9 codes for chronic conditions not updated or discussed during the visit? The problem list is just being reviewed by provider and then the codes for those chronic condition codes added to unrelated visit.
Any input expertise or guidelines appreciated.

Bottom line, if the condition was not addressed and does not affect his MDM then it should not be coded

Per ICD-9 guidelines
Chronic diseases
Chronic diseases treated on an ongoing basis may be coded and reported as many times as the patient receives treatment and care for the condition(s)

I see physicians just go down the laundy list of of conditions in the patient's problem list and just document "stable", or "followed by PCP" etc. They think it is building their MDM and level of E&M. It's not
