Wiki Coding complex closure with a fasciotomy???

HELP! I can't seem to find an up-to-date finite answer involving complex closure coding with fasciotomies and myocutaneous flaps. I code for plastic surgeons and find that they bill for complex closures on most all of their surgeries and insist on billing for them (insurances deny it 95% of time). AAPC coder hits CCI edit when I enter both codes together and mod 59 would not be appropriate - MD's have older reference material indicating simple & intermediate closures are included in fasciotomies but they CAN bill for complex closure? It seems to me both these procedures inherently require a complex closure due to complexity of procedures themselves. Does anyone have any documentation to support this. Exhausted looking on line & finding nothing. Thanks for any help:confused:
