Can we code both E78.5 and E78.00, when both are mentioned in the medical report?

You wouldn't code them together. Cholesterol is a type of lipid.

If the provider diagnosed pure hypercholesterolemia, you would code that. It is more specific than hyperlipidemia, unspecified.

If both types of lipids (cholesterol and triglycerides) are high and it is documented that way, you'd code E78.2 (mixed hyperlipidemia).
 
Thank you, I am also coding the same way and gave the same explanation. There was an audit and the explanation given was that the code description says both the codes are unspecified
E78.00
Pure hypercholesterolemia, unspecified
E78.5
Hyperlipidemia, unspecified
