An echo completed during pregnancy identified the fetus with small VSD. Mom presents to L&D in labor. The H&P shows physician documentation acknowledging the fetal small VSD condition noted on a prior echo. Within the echo report there is documentation "does not appear hemodynamically significant". However, the attending documents "Recommend neonatal ECHO" in the H&P.

Would it be appropriate to code Q21.0 (Ventricular septal defect) on the mother's claim despite the condition referring to the fetus, given the condition was addressed in the mother's MR, prior records were reviewed, and there is documented recommendations?
For that matter, should code O35.8XX1 (Maternal care for other (suspected) fetal abnormality and damage, fetus 1) also be coded?

I really appreciate any input here as I'm losing points for thinking this was appropriately coded.
 
Hi, no.. Q codes are congenital conditions and therefore only used on the patient; in this case, the baby. Please note that for single gestations the correct 7th character is 0, not 1, and cardiac anomalies would be O35.BXX0
