I know that, normally 77002 for fluoroscopy is reported with 64640, destruction by neurolytic agent; other peripheral nerve or branch. But, if the nerve being destroyed is "L5/S1 dorsal rami pain sensory fibers including fine branches to the SI Joint", would it still be appropriate? The June, 2008 CPT Asst. Update says "Code 77002, Fluoroscopic guidance for needle placement (eg, biopsy aspiration, injection, localization device), is intended to be used to report fluoroscopic guidance during injection procedures when fluoroscopic guidance is required in the performance of needle placement in areas other than the spine. I would appreciate a little clarification.
