7-20-09 for office visit and then on 7-22-09 patient's family ask that he document HPI; PMH; ROS; and all reasons and treatment course suggested or admission to nursing facility that this physician does not visit. He won't be seeing the patient any more. Is there a code to bill for just the documentation he did to have patient admitted and transferred care?
