The correct code for the encounter code for blood work is Z09. Then the history code for the Non-Hodgkin's lymphoma or the Hodgkin's lymphoma. If they come in for a port flush it is Z452. Make sure it is no longer active, in compete remission and look closely at your note's. Or query the physician. This took a lot of studying and I went through my school to find this code.

You CANNOT put a history code as a primary diagnosis!!!
