When your provider is reporting on a non-OB pelvis ultrasound, and ovaries are unable to be seen, would the documentation "scanning of the adnexa did not reveal the left ovary" suffice a complete exam, even though the reason they are not seen was not stated? I have been informed by several radiologists that many times the reason they cannot be seen is unknown, so listing a reason would be guessing. What verbiage might a provider use to document that the adnexa was scanned for the ovary, but it was not seen for an unknown reason?
