Wiki Coding from an EMR

We are a very small rural healthcare clinic switching to EMR soon. Right now I code from a charge ticket. When we go to Encite I'm totally lost as to how I will code and what I will code from. Someone said the documentation is sent to the coder, then to the biller then sent out electronically for payment. Just not sure what to expect. Thanks.

you should always code from the documentation never from a charge ticket. You will review the office note and determine the correct codes for the claim.
