does anyone know if it is mandatory to bill G0278 iliac angio along with cath 93458 if done since there is no separate payment for outpt hospital ? also is it only for medicare requirement or other insurances also? I am hospital cath lab coder and I am getting denials when I bill G0278 along with cath 93458 etc codes . I don't recall from what insurance I got denial because I had stopped billing G0278 but now HIM dept wants me to start putting that code in so wanted to clarify if anyone is going thru same problems
