When coding unspecified gout, do I need to first code the anatomical location of the pain? For example, the patient came in with left wrist pain and the provider diagnosed him as gout. Would I first code the wrist pain and then unspecified gout or do I just code the gout?

If pain is a symptom of gout (usually that's how it presents) then no pain would not be coded. ICD 101 - code to definitive diagnosis for outpatient claims
