I am looking for where it says what you can look at while coding, this is out patient. Ex. where does it say we can only look at the progress note from which we are coding? I heard this but have never seen it in writing. Also where does it state we can not look at the x-ray to help with a diagnosis.

Those kinds of coding rules are outlined in the AHA Coding Clinic, and also AHIMA has a number of resources online with regards to the legal medical record. The Coding Clinic is not free; it's available by subscription, but it is the best resource for correct coding out there.
