Wiki Coding hand notes

We have a new doctor to the practice who hand writes some of his exam notes rather than dictating and they are very difficult to read. Is there rules out there when it comes to auditing, that if an auditor can't read the hand notes that the claim can get down coded because of lack of readable data?

If the documentation is not legible then the information cannot be coded. It must be readable to all.
