Can someone help me code the following:

1. Bilateral foraminotomies at the C4-5, C5-6, and C6-7 level.

2. L2-3, L3-4 TLIF's with complete left-sided facetectomies, annulotomies, diskectomies, and placement of PEEK interbody grafts, as well as pedicle screws bilaterally at the L2 and L3 levels. The L4 pedicle screws were removed and new pedicle screws were placed at the L5 of a bigger diameter with titanium rods and locking end caps and cross-link.

3. Use of intraoperative O-arm with Stealth guidance.

4. Intraoperative steroid injections bilaterally at C4-5, C5-6, and C6-7.

Sorry there is a lot going on here. Any help would be really appreciated.

I would do this:

Plus whatever kind of graft was used.
You cant bill for the new screw because there was an extension of the fusion.
For the L2-3, L3-4 facetectomies, you can only bill for that if it was additional decompression work done beyond that needed for the TLIF. If so I would bill 63048x4
