Patient with malfunctioning dialysis access of the Rt arm
Procedures done are Fistulagram - 36147 percutaneous balloon angioplasty of intra access 35476/75978 and insertion of 8 mm gortex graft ? 37238

You may consider codes 36832 or 36833 for open AVF revision - without thrombectomy and with thrombectomy, respectively. One of these might be appropriate for the " insertion of 8 mm gortex graft" but I can't be sure without the complete procedure report.
