Wiki coding help with cervical decompressive laminectomy

How would you code this? C3 through C7 deompressive laminectomy, right C56 foraminotomy and nerve root decompression.

The note reads laminectomy was carried out, c3 thru c7. A generous right c5-6 foraminotomy was carried out as this was the most symptomatic level.

Would I code this 63015 & 63045-59 or
63015-22 and increase the fee due to the extra work at c5-6?

I could use come advise on this if someone could help me out.
