Hi All,

Doctor performed Hemorrhoidectomy, Fissurectomy and Lateral Sphincterotomy.

There combination code for Hemorrhoidectomy and fissurectomy- 46257
Can anyone clarify me that Sphincterotomy is inclusive in this code or not?

Or

Can I use Hemorrhoidectomy - 46255 and

Fissurectomy with or without sphincterotomy- 46200?

Edit says 46200 is allowed with 59-Modifier with 46255.

Please guide in which occasions we can use these codes and how should be the documentation.
