Need help coding this note,

84 year old genleman was found to have multiple skin lesions on face suggestive of malignant condition. At left preauricular area 13mm red, elevated skin lesion suggestive of squamous carcinoma and the second to the left lateral neck 17mm red irregular surface suggestive of skin cancer of squamous cell carcinoma and both existed there for 3 years. The third lesion was located to the right cheek measuing 7mm with a white scaly area suggestive of solar keratosis persists with squamous cell carcinoma. Treatment is indicated.
Incisional biopsy fo the neck and anterior preauricular lesions closing with fiber nylon sutures. The right cheek was completely excised out by doing excisional biopsy and then closed with nylon in running fashion.

There is no Path report attached.

Can someone help please.
