Since hot packs 97010 is a bundled code for outpatient therapy and therefore not reimbursable, when is it reimburseable?

I also have understood that hot pack time can not be counted in MDS time for Med A RUG as it is a nonskilled service, but I have also read that because it is a bundled service the time can be counted for Med A. Does anyone have specific CMS references?
