Wiki Coding Hypertensive heart and CKD 404.XX

Need clarification if it is appropriate to use the combination code 404.xx when a patient has CKD with hypertension and also has congestive heart failure (again dx of HTN is listed separately). Can you code 404.xx this if the HTN and CHF are not listed together or stated as hypertensive heart disease or should they be coded separately?:confused:
 
just a suggestion

code the CHF first and with the fifith digit. Then as a result of the HTN with the XX include the CHF from HTN. And then code the 404xx because it describe the stage of renal disease as the secondary to the cause of the HTN the HTN brought on all the other condition unless the patient already had renal complication.
