would a returning patient on chemo be coded with T45.1X5A or would we use D for subsequent?
 
The T45 codes are for reactions to the drugs, if you are treating the reaction with active treatment then yes, if the patient has had no poisoning or adverse effect to the chemo agent then you will not use these codes at all.
