Wiki coding J0665 and J3301 for TPI's done in the office

When we code for a TPI 20553 and we use 3 cc of the Bupivacaine (code J0665) and 1 cc of the Triamcinolone (code J3301) how many units should we code the J0665 and J3301?
 
We have our doctors dictate the mg’s or ml’s depending on the description from the cpt code. So J6605 being 0.5 mgs. If they dictate 2 mg’s we would bill x4 units. J3301 is per 10 mg’s (double check the description for this) if they dictate 40 mg’s we would bill x4 units. I’m still trying to figure out if we can even bill for the bupivacaine or if it’s included in the trigger point/ injection/block if done for therapeutic reasons.
