We have patients that are on pain contracts and as part of that, we required a UDS to determine if they have the med (in this case methadone or oxycotin) in their system and if it is at therapeutic levels.

If the patient only comes in for lab work, do I only code:

1) Therapeutic Drug Monitoring (V58.83)
2) Long Term use of other med (V58.69)
3) What ever the condition that they receive the meds for - chronic pain for example
4) Lab exam (V72.6)

Or only, #1, 3 and 4?

You need at a minimum #1 and #2, If you look in the book the V58.83 code states to use an additional code from the V58.6 category for the drug being monitored. I always code #1, #2, and #3 but never #4.
 
I failed to mention that I work in an outpatient facility and often times patients come in only to see the lab department and no one else, that is why I put in #4.
