Does anyone know, in order to code multiple Breast biopsies, is it based on the number of incisions or each mass/lesion? Example: Lt Breast 2 o'c middle depth and 2 o'c posterior depth.
Breast biopsies are coded per lesion.
Lesions in the other
breast are additional lesions when performed with the same type of imaging guidance.
Multiple number of core samples or needle passes in same lesion are coded as single CPT.
e.g.
A patient with a 2 cm lesion in the 2 o’clock position of the right breast. After careful sterile prep and drape, a
14 gauge needle is placed in the lesion in the right breast under sterile ultrasound guidance and 4 core samples
obtained (19083).
Patient with non-palpable lesions at 2 o’clock and 10 o’clock of the left breast. Both lesions are localized
using MR guidance and biopsied with percutaneous vacuum-assisted technique (19085, 19086).
If multiple lesions are biopsied using different methods of imaging guidance (e.g., ultrasound
guidance for first lesion and MR guidance for second lesion), report the first lesion biopsied
with a distinct imaging modality with the code describing an initial lesion. In the example
given, codes 19083 and 19085 would be reported
