That's how we do it here. The medical records clerk assembles the charts and prints a deficiency report on top of the chart if one is needed (for signatures, etc.). We code the charts and then return them to her so she can send them where they need to go for whatever reason. I only code charts that are complete, meaning they have all of the proper paperwork neede, ie. H&P, D/S, orders, etc. The signatures never make a difference to us here, coding wise.
