h/o Graves Disease but currently hypothyroid would that be 2 codes Z86.39 & E039?

To address the history of Grave's Disease Z86.39 would be correct. If the patient currently has hypothyroid, then you would code that as appropriate based on the type of hypothyroid disease. 2 codes would be appropriate.
