I am looking for coding guidelines for coding from the in-patient medical record or any progress note.
Example- Pelvic x-ray was rendered 9/14/19, indication rule out bone pain (result were negative). OB patient admitted 09/12/19 - 09/15/19. I am having difficult time obtaining the order since we only render the professional component. Progress notes dated 9/12/19 & 09/14/19 DO NOT state any type of pelvic pain. The progress note dated 09/15/19 (after the procedure) mentions pelvic pain.
I am looking for the guidelines where it states which progress notes can be used to code the procedure. Is there a date range we are only allowed to use?
