Wiki Coding Re-Admissions to SNF's

If the patient in a SNF was discharged or admitted to the hospital and then re-admitted to the same SNF can I code this as a new patient encounter - 99304, etc. Also, if the patient is transferred with or without a hospital stay or went home, and then admitted to another SNF can I code this as a new encounter? Confused!
 
Ben-So then I am NOT able to use a 99304 if it is considered an established patient.

My physician believes that he can use the discharge from the hospital as his discharge from the SNF and then when the patient returns to the SNF he can treat him like a new admission back to the same SNF and use the Admission from the hospital stay as the admission back to the nursing home.

Make sense :confused:?
