I received these two questions from a physician and I think I know the answer but cannot find clear documentation on it. Please help!!

1. If the patient in a SNF was d/c'd or adm to hospital, then re-admitted to the same SNF, do I code a "new" pt encounter, e.g., 99306??
2. If the pt is transferred, w/ or without a hosp stay (or went home, then admitted), to ANOTHER snf, do I or may I code it as a "new" pt encounter, i.e., 99306??

My understanding is they are not considered a new patient if they have been seen in the last 3 years by that office. However because it is a nursing home they can reuse the 99306 when readmitting correct?
