I have an order for an OB US for the mother who is at approximately 20 weeks. The dx on the order 764.90 IUGR. I cannot use the numeric portion of the code because that refers to the fetus. The code that it needs to be is 656.53 poor fetal growth, affecting management of mother, antepartum. If you go to to growth, both codes are there, they chose the wrong numeric part but correct narritive part.

My question is this: what is the rule when the Numeric portion of the diagnosis does not match the narrative portion of the diagnosis? Can one code just from the narritive portion and ignore the numeric portion of this or is a new order needed?

Thanks for any input on this matter!!
