When coding a surgery how do you know which procedure is primary. Do you go by the order of the doctors notes or do you go by the order of the diagnosis?

Neither. Since there is a multiple surgery/procedure reduction rule for payment you want to list them in order of rvu. You don't want a 50 percent reduction in payment on your highest paying code.
