I'm an HCC coder. Where I work we are having discussions on how to code certain dx when there is a "/" between 2 diagnosis. For example:

COPD/Emphysema -- Do we code the emphysema because of higher specificity? Do we code neither because it could be either code and we are just not sure which? I think we should code the lower specificity code, COPD to err on the side of caution. We don't have the ability to really query the provider as we are HCC coders for a large company.
