Hello,

(This is for NICU dx and claims) If a Physician does not specify and document a dx code on the medical chart, should the code be denied? P84 is what is billed. The clinical documentation does not list the P84 and does not link it to anything. The assumption being made is the P84 is for respiratory acidosis at birth. The provider does not list this in the problem list nor is there an assessment or treatment plan.

I work for a vendor who does claims reviews for Healthcare companies. The P84 code has been giving us a very difficult time because I have yet to see one example of it being correctly coded. I think it should be outright denied because it is not linked to anything.

Would love to get some feedback from everyone.
