Wiki Colonoscopy: Can I use K6389 (Other specified diseases of the intestine) with 45388, with a polyp documented?

Would code: K6389, be acceptable for both sides, facility only or can I use it at all, because the physician documented polyp, not lesion?
I cannot use dx: K635 (default for colon polyp) on the Facility side, but I could use it on the Physician side.

1. Two sessile polyps were found in the rectum and ascending colon. The polyps were 6 to 7 mm in size. These polyps were removed with a cold snare. Resection and retrieval were complete. CPT-45385, DX-D128, D122
2. A 2 mm polyp was found in the sigmoid colon. The polyp was sessile. Fulguration to ablate the lesion by bipolar probe was successful. CPT-45388, DX: ? (K635 or K6389)

I hope this makes sense, I would appreciate your comment.
