Code the secondary neoplasm first, then use 199.1 for the primary, since that is the unknown.

Wouldn't you use a V-code first since it is being ruled out and not as of yet confirmed?

Do not use 199.1 for the unknown primary.
With a neuroendocrine tumor use 209.20. The met will also be a code from the 209.7* category.
As for the V code... it would depend on if it was ordered/documented as a screening or a diagnostic colonoscopy.
