Medicare local coverage determinations
I strongly suggest you familiarize yourself with you Medicare MAC. Go online to their website and look up the local coverage determinations for colonoscopy. This is their entire policy on what is covered/not covered and what diagnoses support doing them. Every MAC has their own policies so it wouldn't make sense for me to tell you what my MAC policy is.
