Comanangement surgery

How do I code for comanagement surgery? My doctor will be performing the surgery but another ophthalmologist will be doing the follow up care. I know there is a modifier but I would like some input regarding this. Also, is there seperate reimbursement for comanagement surgery?
 
You will be using a 54 along with which eye modifier for the surgery. The physician performing the post operative care will use a 55 modifier.
