How do you interpret the new codes Z17.4- Combined Receptor Status? Such as do the providers need to document as such (HR+/-) or is stating at least one receptor is positive or negative sufficient? We are having a difference of opinion within our office as some feel the documentation needs to state HR+/- as that is how it can be found in the index whereas others feel that by stating a hormone receptor(ER/PR) as either +/- is sufficient.

What would you expect to be in the documentation to code as Z17.4-
For example, are any of the below acceptable to use Z17.411 Hormone receptor positive with human epidermal growth factor receptor 2 negative status
  • ER+, PR-, HER2-
  • ER/PR+, HER2-
  • HR+, HER2-
