Wiki combining ED encounters and increasing the E&M

we are being asked to combine ED encounters and increase the E&M levels for pts that have Medicaid who are seen twice in the ED on the same DOS. Example Pts first visit is a level 2. pts second visit is a level 3, we are being asked to combine the visits and charge a level 4. I cannot find any information regarding this. Can anyone tell me if this is ok to do.
