I have a provider who has dictated 2 operative reports. On the same day the patient was taken to the cath lab and had a coronary angiography procedure that had to be stopped due to a spasm and patient already had received Angiomax. He planned to take back to cath lab after Angiomax wore off for PCI to the LAD.

Later that day patient was taken back to the cath lab and had stent placement to the LAD. Before leaving the lab the patient had left ventriculogram since it was not done as a part of the coronary angiography earlier in the day. Catheter was advanced across the aortic valve and the left venticulogram was performed.

My question is can these two operative reports be combined and a LHC and stent be billed since this was same provider, same date of service, just different sessions?

Any help and direction would be appreciated.
