Wiki commercial primary, Medicare secondary

If a person has Commercial BCBS as primary insurance, and Medicare as a secondary insurance. Can a surgeon bill a consult code?

Id just go with Office visit. Medicare doesn't take consult and many BCBS don't allow either.

If Blue Cross covers a consult, go ahead and bill as a consult. When Blue Cross pays, convert the consult to a comparable office visit code and transfer to Medicare. It has been a few years since I have billed consults, but if I remember correctly, that is the proper procedure.
