Does anyone know how to correctly bill for a physician's time at a commitment hearing?

We bill ours out like a deposition with a misc code and a fee we determined based on time

Who do you bill it to, patient, insurance, attorney?
Are they paid?

we bill depositions to the attorneys office. I guess I cant say for sure on a commitment hearing.. I misread earlier.

Who is requesting dr to be there? I would bill that person and I would have it paid before he does the service.
