Provider did an excision of an irregular lesion that was on top of a scar from previous shave. The lesion was a blue nevus. The provider first undermined on either side of the ellipse and then did a 2 layer closure. Questioning whether this would be complex or intermediate closure. Provider says complex. Thanks

CPT guidelines

When in doubt, refer to your CPT guidelines.

From the 2014 Professional Edition of CPT, published by AMA, pg 75:
Complex repair includes the repair of wounds requiring more than layered closure, viz., scar revisions, debridement .... extensive undermining, ...." (emphasis added by FTB)


If your documentation supports either scar revision or extensive undermining, then it can be considered a complex repair.
