The surgeon documented a complex total knee replacement because of severe arthritis and more stiffness which made the exposure challenging. Is this common or should 27447 cover it?

If documentation states a significant amount of time was spent to complete the procedure and specifies the time, it can be appended modifier 22. Please make sure there must be a proper description of the extra work done in addition to usual to support mod 22. Also, attach OP report with claim.

Modifier 22 is for the physician, if your coding for the facility, it cannot be used.
