I am looking for resources that show that documentation needs to be reviewed to the charges before entering. Can anyone point me in the right direction?

I'm not certain I understand what you're asking. Who said documentation needs to be reviewed if the chart is already coded? Who would be reviewing? Where are the charges now if not entered?
