does anyone no of a dx code for concentration down. I thought 314.00 attention deficit disorder without hyperactivity would cover that.

I'd say no.

ADD is a concrete diagnosis and I wouldn't recommend it for a decrease in concentration. It may be nothing to do with clinical ADD and you'd have assigned a diagnosis for something the patient didn't have. Many neurotic and personality disorders include concentration difficulties. I would ask your provider for clarification to see if a mental health diagnosis applies. Otherwise, I'd stick with something benign like 780.99 (other general symptoms) or maybe 780.97 (altered mental status--but only if the provider agrees this fits).
 
I agree with Belinda. I would not assign anything from the mental disorders section without clarification from the physician.
