We recently discharged a patient from our psychiatric inpatient facility to a SNF. We used discharge status 03. The Medicare Advantage plan rejected the claim for a condition code. Can anyone suggest what code(s) we need to use? Further research into this stay revealed patient was inpatient at ACH discharged to a rehab facility. Then after 14 day stay there was discharged to emergency room for suicidal ideation and then from emergency room was admitted to our psychiatric inpatient facility where they were for 24 days and discharged to SNF. Also there is a primary auto insurance for accident that occurred prior to ACH admission although admission to our facility was not related to accident.
We rarely have inpatient claims this complicated so any help would be appreciated.
